How it works
Complete your request online, meet with a HawkFMLA clinician by secure video, and receive your completed documentation electronically.
Step 1 — Complete Secure Intake
Complete the secure online intake, provide information about your medical condition and leave request, upload any available employer paperwork or supporting medical records, and submit the $179 review fee.
Step 2 — Schedule Video Visit
After your intake and payment are received, you will receive a confirmation email with a link and access code to schedule your required secure video visit.
Step 3 — Video Visit & Clinical Review
At your brief secure video visit, your HawkFMLA clinician will review your information and discuss your condition, how it affects your ability to work or provide care, and the type of leave being requested.
Step 4 — Completed Documentation
After the clinical evaluation is complete, your HawkFMLA clinician will complete appropriate FMLA medical-certification paperwork and securely return it to you with instructions for submitting it to your employer.
Submission and payment do not guarantee that requested leave can be medically or legally certified. Certification depends on the HawkFMLA clinician's independent medical assessment and the information provided. Additional records or evaluation by your treating clinician may be required.
