Refund & Cancellation Policy

Before Physician Review Begins

If you cancel your request before substantive physician review has begun, you may request a full refund.

Please provide your name and enough information for us to identify your request. Do not send detailed medical information through ordinary email or fax.

After Physician Review Begins

Once a physician has begun substantive review of your intake, medical records, employer paperwork, or other documentation, the professional service has begun.

At that point, the fee is generally non-refundable, even if:

• the physician cannot medically support the requested certification;
• the physician recommends different leave dates or restrictions;
• additional information is required;
• the patient decides not to continue; or
• the employer ultimately denies the leave request.

The fee is for physician review and professional medical judgment, not for a guaranteed certification or employer approval.

Insufficient Information

If additional information is required to complete the review, we will ordinarily attempt to contact you.

If you do not provide requested information within a reasonable period, the request may be closed.

A refund is not guaranteed if substantive physician review has already occurred.

Duplicate Payments

If you are charged twice for the same request because of a payment or technical error, please contact us.

Verified duplicate charges will be refunded.

Technical Problems

If a technical problem prevents us from receiving or reviewing your request and the service cannot reasonably be provided, we will work with you to correct the problem or provide an appropriate refund.

Employer Denial

A refund is not provided solely because an employer, insurer, leave administrator, or other third party denies FMLA leave or refuses to accept documentation.

HawkFMLA does not control employer eligibility or approval decisions.

Physician Unable to Certify

If the physician determines after review that the requested certification is not medically supportable, the fee generally remains non-refundable because the physician review service has been performed.

Exceptional circumstances may be reviewed individually.

Refund Processing

Approved refunds will be returned to the original payment method whenever practicable.

Processing times may vary depending on the payment processor and financial institution.

Chargebacks

Please contact us before initiating a payment dispute or chargeback so we have an opportunity to investigate and resolve the issue.

This provision does not limit any legal rights you may have regarding disputed transactions.
